When you are experiencing a serious illness, you have the right to choose your own care.  The more you understand what is happening with your health and what your care options are, the better choices you can make.  A MyMichigan Home Care social worker or nurse is available to help you consider your goals, values and priorities and determine the most appropriate services to meet your needs. This free informational visit is provided in the comfort and privacy of your home and can be scheduled to accommodate others from your support system as desired. Counties served include Alcona, Alpena, Arenac, Bay, Clare, Crawford, Gladwin, Gratiot, Iosco, Isabella, Midland, Montmorency, Ogemaw, Oscoda, Otsego and Presque Isle, as well as portions of Cheboygan, Mecosta, Montcalm, Roscommon and Saginaw Counties.

Who Can Benefit?

People who may benefit from MyMichigan Home Care's services include:

  • Those recovering after surgery or hospitalization
  • Those with a recent change in health status that limits mobility or necessitates more specialized care

  • Those requiring some assistance to remain at home
  • Those that wish to explore discontinuing curative treatment and/or hospitalizations
