Home Health Care services are for homebound people who need skilled nursing or therapy. Services are ordered by a health care provider and care is delivered in the comfort of your home. Nurses, aides and/or volunteers come to your home to treat an illness or chronic condition. Our goal is to get you better so that you can regain your independence and become as self-sufficient as possible.
MyMichigan Home Care provides skilled home health services throughout middle and northern Michigan (Alcona, Alpena, Arenac, Bay, Clare, Crawford, Gladwin, Gratiot, Iosco, Isabella, Midland, Montmorency, Ogemaw, Oscoda, Otsego and Presque Isle Counties, as well as portions of Cheboygan, Mecosta, Montcalm, Roscommon and Saginaw Counties). Home care can help patients avoid admission to a hospital or nursing home.
Some services we provide:
- Intravenous therapy – includes administering medications, managing venous access devices and drawing samples for laboratory tests
- Oncology nursing – includes chemotherapy, symptom management, nutritional assessment, and helping patients and families cope
- Cardiac recovery – includes testing and monitoring, exercise supervision, education, registered dietitian services, and preparation for outpatient cardiac recovery program
- Enterostomal therapy – includes wound care and follow-up care for major bladder or intestinal surgery
- Maternal/child care – includes education about caring for newborns and follow-up treatments such as phototherapy for infants with jaundice

Hospice is a home care program for terminally ill individuals (prognosis of six months or less) and their families. It is appropriate for those who are no longer pursuing treatment. We recommend starting hospice care earlier in the disease process rather than waiting until the final moments of life, as this maximizes opportunities to improve the patient’s quality of life and ensure their wishes are honored.
Hospice provides clinical management of symptoms and pain, enabling you or your loved ones to live as fully and comfortably as possible while remaining in the familiar surroundings of home. The focus is on caring, not curing.
MyMichigan Hospice's services can be provided at home whether you live in your own home or at a friend or family member's home, a nursing facility, an assisted living center or a foster care home.
As an alternative to care at home, MyMichigan Hospice offers the Woodland Hospice House, located in Mt. Pleasant, Michigan. The Woodland Hospice House is a beautiful eight-suite facility that focuses on privacy, comfort, and individualized care to honor your and your family’s wishes.

Palliative Care is comprehensive care that focuses on relieving pain, symptoms and stress for those who are living with a chronic, debilitating or serious illness. The goal is to improve the quality of life for both you and your family.
Anyone at any age and at any stage of illness can request palliative care services alongside their treatment plan.
MyMichigan Health offers a home- and clinic-based Palliative Care program serving Clare, Gladwin, Gratiot, Isabella, Midland and Montcalm counties, as well as portion of Arenac, Bay, Mecosta, Ogemaw, Roscommon and Saginaw counties.
