Continuing Care Clinics: Supporting You Through Transitions in Care
MyMichigan’s Continuing Care Clinics are here to support you during major transitions in your health, such as after discharge from a hospital, Emergency Department or long-term care facility, or while you are establishing with a new primary care provider. When you’ve been treated for a serious illness, transitioning back home or to another level of care can be challenging. A follow-up appointment can answer your questions and get you on the path to feeling better. Your follow-up visit is important to your recovery. We want you to be seen without delay! If your primary care provider doesn’t have an opening in the recommended time frame, or if you don’t have a primary care provider, we may refer you to the Continuing Care Clinic. In many cases, we may help you schedule this appointment before you even leave the facility.
What We Do
- Guide you through the steps of your recovery and bridge the gap until your primary provider can resume your care.
- Review your diagnosis, treatment plan and instructions to make sure you understand them and don’t have any obstacles.
- Review your medications and make any needed changes for better results, lower costs or fewer side effects.
- Arrange other follow-up care you may need, such as specialist visits, tests or procedures.
- Find resources to help you with things like food, transportation or household chores during recovery.
- Work with your primary care provider to resume your care, or help you find a primary care provider if you don’t have one.
Our team includes physicians and advanced practice providers, patient care navigators, nurse care managers and ambulatory pharmacists. You may see any or all of these team members depending on your medical needs.
Video Visits Also Available
Our clinics are located in Alma and Midland, but we can also provide video visits if travel is a challenge for you and if an in-person exam is not necessary. If this is the best option for you, we’ll provide you detailed instructions for how to connect.
