Sports Physicals

Picture of Multiple SportsSports physicals play a key role in assuring the safety of our athletes. Ideally, this evaluation should be performed at least six weeks prior to the season in order to allow time for the athlete and their health care provider to identify and resolve any concerns that might affect their sports participation.

Please download the Sports Physical form, complete the parent/athlete section and take to your health care provider for completion. The completed and signed form then needs to be returned to the school.
