Eligibility and How to Apply

Application Requirements

  • A Doctor of Pharmacy (Pharm.D.) degree from a college of pharmacy accredited by the Accreditation Council for Pharmacy Education
  • A pharmacist license or be eligible for licensure in the state of Michigan
  • Interested applicants must submit the standard application materials through PhORCAS by the deadline of January 1, which includes:
    • Letter of Intent
    • Recommendations from three professional sources
    • Copy of your College of Pharmacy transcripts
    • Copy of your Curriculum Vitae
    • Participation in the ASHP Residency Matching Program 
  • MyMichigan Health Midland ASHP Match #288213

Additional Requirements

Pharmacy Residents must meet all hiring policies set by MyMichigan Health including but not limited to:

  • Immunization
  • Physical Health Review
  • Drug Screening
  • HIPAA training, safety training, and any other training deemed necessary by MyMichigan Health
