Flight Physical Examinations

To fly a plane a pilots license and a medial certificate, which involves a physical from a Federal Aviation Administration (FAA) Certified Medical Examiner (AME) is required.

MyMichigan Health offers FAA medical certificates for Class II and Class III authorized flight physical examinations through the WellSport office. To obtain a flight physical, follow these steps:

Scheduling Your Medical Examination

  1. Call the WellSport office, (989) 837-9350, to schedule a flight physical. This office handles may types of appointments, please inform staff that you need to schedule a Flight Physical.
  2. Register and complete FAA form 8500-8 through the FAA MedXPress system.
  3. Please print your completed application and bring it with you to your appointment. If you do not have this paperwork with you, your appointment will be cancelled as you will be accessed a missed appointment fee of $25.
  4. Review all necessary documentation in regards to medical conditions. FAA Medical Certification or www.flighphysical.com
  5. If you have any of the following conditions, bring all Conditions AMEs Can Issue (CACI) worksheets and supporting documentation with you to your appointment (i.e. primary or specialist provider note, up-to-date blood or urine results, list of medications, etc). All questions must be answered by supporting documentation. It is acceptable for you to have your primary care provider complete the CACI form with his/her signature and information or in a letter on office letterhead.
  6. If you have a waiver or special issuance, please bring that to our appointment.

He is also an authorized Aviation Medical Examiner (AME) by the Federal Aviation Administration to provide Class II and Class III flight physicals. In addition to providing flight physicals, Dr. Diaz provides sports injury evaluation, treatment and preventive care to athletes of all levels and ages through MidMichigan Health’s WellSport Program. He serves as a flight surgeon in the National Guard.
