MyChart Proxy Access for Minor Patients (Patients Under Age 18)

Parents and legal guardians may be eligible for proxy access to their child's MyChart account, depending on the child’s age and whether the child is legally emancipated. These limitations do not affect any legal rights you have to access your child's medical records by other means. Proxy Access Levels

• Ages 0-10: A parent or legal guardian may activate their child’s account and is eligible for full proxy access.
• Ages 11-17: When a patient turns 11, the parent or guardian’s proxy access will automatically be disabled. Patients ages 11-17 may request an adolescent account by applying in person at their participating provider's office. We encourage you to first discuss with your doctor whether an adolescent account may be right for you. Patients with adolescent accounts may decide whether to grant proxy access to a parent or guardian and may revoke proxy access at any time. This is required to ensure that certain parts of the minor's record remain protected in accordance with Michigan law.
• Ages 18 an older: Access to adolescent accounts will automatically be disabled when the patient turns 18. The adult patient may then apply for full access to their own account.
• Emancipated Minors: The child is eligible for full access to their account. They must provide proof of legal emancipation. The child may delegate proxy access if desired.

Full proxy access enables you to:

• View all information in the patient’s MyChart account
• Perform tasks on behalf of the patient, such as appointments, prescription renewals, and secure messaging
• Grant or revoke proxy access

Delegated proxy access enables you to:

• View information in the patient’s MyChart account
• Perform tasks on behalf of the patient, such as appointments, prescription renewals, and secure messaging

Why is proxy access disabled for adolescent accounts?

Under Michigan law, minors have the right to receive certain types of medical treatment (listed below) without the consent or notification of their parent or guardian. If the minor lawfully obtained health care without the consent or notification of a parent or guardian, the minor has the exclusive right to release information relating to that care. (MCL 333.26263(n))

• Prenatal or pregnancy-related care. (MCL 333.9132, 45 CFR 164.502)
• Care for a minor child of the minor. (MCL 333.9132; 45 CFR 164.502)
• Venereal disease or HIV. (MCL 333.5127; 45 CFR 164.502)
• Substance abuse. (MCL 333.6121; 45 CFR 164.502)
• Mental health. (MCL 330.1707)

We currently do not have the ability to filter these specific types of information out of the MyChart account. To ensure those portions of the minor's record remain protected, we enable adolescents to manage proxy access on their own behalf. The parent or guardian may still request access to their child's medical record by applying in person or by mail.

How to Apply for Proxy Access

For children ages 0-10, you may apply for proxy access by completing Form EMR04140 and mailing it or faxing it to:

Or you may take it to the participating provider's office or Medical Center where your child receives their care. If you are not the biological parent, you will also need to include copies of the Court Order Appointing Guardian and Letters of Guardianship verifying your status as permanent legal guardian.

Patients ages 11-17 may request an adolescent account by applying in person at their participating provider's office. Once their account is activated, they will have the option to grant proxy access if desired.
